    目的 探讨汉族人群中腺苷酸环化酶-2(ADCY-2)基因rs326174 位点多态性与双相障碍 患者改良性电抽搐治疗(MECT)疗效之间的关系。方法 入组100 例双相障碍患者,随机数字表法分 为MECT 组和非MECT 组各50 例,使用TaqMan 基因分型技术检测ADCY-2 基因rs326174 位点多态性, 采用大体评定量表(GAS)评估疗效,对所得结果进行统计分析。结果 两组患者均检测出A、C 两种 等位基因,共AA、AC、CC 3 种基因型,基因型分布无差异。基线时,两组间GAS 评分差异无统计学意 义(P> 0.05);而治疗后第1,2,4 周末,MECT组GAS 评分均显著高于非MECT组,差异有统计学意义 (P<0.05)。MECT组内比较,基线时和第4周末,不同基因型的患者GAS评分之间差异无统计学意义(P> 0.05);第1,2周末,3组基因型患者间GAS评分比较,AA型>AC型>CC型,差异有统计学意义(P<0.05)。 结论 MECT 是治疗双相障碍的有效方法,ADCY2 基因多态性可能与MECT 治疗的疗效存在关联性。

    Abstract:

    Objective To investigate the association of the effect of modified electroconvulsive therapy with polymorphism of rs326174 loci in ADCY-2 gene in Chinese patients with bipolar disorder. Methods A total of 100 patients with bipolar disorder( BPD) were distributed into MECT group and non MECT group by random number table method with 50 cases in each group. The polymorphism of rs326174 loci in ADCY-2 gene was detected by TaqMan genotyping. The efficacy was evaluated by the General Rating Scale(GAS). The results were statistically analyzed. Results There was no difference in genotype distribution between the two groups. A and C alleles were both detected in two groups. There were three genotypes: AA, AC and CC. At baseline, there was no statistically significant difference in GAS score between the two groups( P>0.05), while GAS score in group MECT were significantly higher than those in non MECT group at the first weekend, the second weekend and the fourth weekend after treatment( P< 0.05). In the MECT group, there was no statistical difference in the GAS score among the three genotypes of patients before treatment and the fourth weekend after treatment (P> 0.05). At both end of the first and second week, the score of GAS in three genotypes from high to low were AA, AC and CC genotype, and the difference was statistical significant( P<0.05). Conclusions MECT is an effective method for the treatment of bipolar disorder. The polymorphism of ADCY2 gene may be associated with efficacy of MECT.
