    摘要:

    目的 评价正念减压疗法在失眠患者中的应用效果。方法 检索Cochrane Library、 PubMed、Medline、EMbase、Web of Science、中国知网、维普和万方数据库等,收集符合纳入标准的随 机对照研究,采用RevMan 5.3 软件进行统计分析。结果 最终纳入8 项随机对照试验,共922 例患者。 Meta 分析显示,正念减压疗法较常规护理提升了失眠患者的睡眠质量(SMD=-1.02,95%CI=-1.21~ -0.83,P< 0.01),尤其是改善日间功能障碍(MD=-0.35,95%CI=-0.41~-0.30,P< 0.01)和睡眠效率 (SMD=-0.88,95%CI=-1.14~-0.61,P< 0.01),并缓解抑郁(SMD=-1.16,95%CI=-1.55~-0.77,P< 0.01) 和焦虑(MD=-7.49,95%CI=-8.23~-6.75,P< 0.01)。结论 正念减压疗法能够提升失眠患者的睡眠质 量,改善日间功能障碍,提高睡眠效率,但对于改善焦虑及抑郁情绪尚需进行深入研究及验证。

    Abstract:

    Objectives To evaluate the effects of mindfulness-based stress reduction( MBSR) therapy in insomnia patients. Methods Cochrane Library, PubMed, Medline, EMbase, Web of Science, China HowNet, Weipu and Wanfang databases were retrieved to collect randomized controlled trails( RCTs) that met the inclusion criteria. RevMan 5.3 software was used for statistical analysis. Results A total of 8 RCTs involving 922 patients were included. Meta-analysis showed that MBSR significantly improved the sleep quality of insomnia patients[ SMD=-1.02, 95%CI( from -1.21 to -0.83), P < 0.01], especially improving daytime dysfunction[ MD=-0.35, 95%CI( from -0.41 to -0.30),P < 0.01], sleep efficiency[ SMD=-0.88, 95%CI (from -1.14 to -0.61), P<0.01], relieve depression[ SMD=-1.16, 95%CI( from -1.55 to -0.77), P<0.01] and anxiety[ MD=-7.49, 95%CI( from -8.23 to -6.75), P < 0.01]. Conclusions MBSR can improved sleep quality, daytime dysfunction, sleep efficiency, relieve anxiety and depression in patients with insomnia. However, further research and verification on anxiety and depression improving are needed.
