    目的 比较视神经脊髓炎谱系疾病(NMOSD)与髓鞘少突胶质细胞糖蛋白(MOG)抗体 相关疾病(MOGAD)患者血清中21 种氨基酸水平,探讨氨基酸水平差异与神经功能残障的相关性。 方法 从国家炎性脱髓鞘病登记队列(NCT05154370)中选取2024 年1 月—2025 年1 月在首都医科大学 附属北京天坛医院神经感染与免疫病区住院并确诊为NMOSD 或MOGAD 的137 例患者为研究对象,其 中NMOSD 患者92 例,MOGAD 患者45 例。应用液相色谱- 质谱联用法(LC-MS/MS)检测两组患者血清 中21 种氨基酸水平并比较。使用Pearson 相关分析氨基酸与NMOSD 患者临床扩展致残量表(EDSS)评 分的相关性。结果 NMOSD 组患者血清中8 种必需氨基酸(亮氨酸、蛋氨酸、色氨酸、苯丙氨酸、赖氨 酸、缬氨酸、异亮氨酸、苏氨酸)水平均低于MOGAD 组,差异均有统计学意义(Z/t=3.116、3.139、2.683、 2.130、2.060、2.411、2.225、2.014;均P< 0.05)。NMOSD 患者血清中4 种非必需氨基酸(酪氨酸、谷氨酰 胺、天冬酰胺、脯氨酸)水平低于MOGAD 组,差异均有统计学意义(t/Z=2.184、2.463、2.805、2.351;均P< 0.05)。NMOSD 患者血清色氨酸、缬氨酸、酪氨酸水平与EDSS 评分呈负相关(r=-0.318、-0.248、-0.242; 均P< 0.05)。结论 氨基酸整体水平偏低是NMOSD 患者神经功能残障较MOGAD 患者偏重的重要原 因之一。关注NMOSD 患者的代谢营养状况,补充必需氨基酸,对改善NMOSD 患者疾病状态、阻止疾 病进展具有积极作用。

    Abstract:

    Objective To compare the levels of 21 amino acids in the serum of patients with neuromyelitis optica spectrum disorder( NMOSD) and myelin oligodendrocyte glycoprotein( MOG) antibodyassociated disease( MOGAD), and explore the correlation between amino acid level differences and neurological disability. Methods From the China National Registry of Neuro-Inflammatory Diseases( NCT05154370), 137 patients who were admitted to the Department of Neurological Infection and Immunology of Beijing Tiantan Hospital, Capital Medical University and diagnosed with NMOSD or MOGAD from January 2024 to January 2025 were selected as study subjects, including 92 NMOSD patients and 45 MOGAD patients. The levels of 21 amino acids in the serum of two groups of patients were detected and compared using liquid chromatographymass spectrometry( LC-MS/MS). Pearson correlation was used to analyze the correlation between amino acids and the Expanded Disability Status Scale( EDSS) scores in patients with NMOSD. Results The levels of eight essential amino acids( leucine, methionine, tryptophan, phenylalanine, lysine, valine, isoleucine, and threonine) in the serum of patients in the NMOSD group were all lower than those in the MOGAD group, with statistically significant differences( Z/t=3.116, 3.139, 2.683, 2.130, 2.060, 2.411, 2.225, 2.014; all P< 0.05). The levels of four non-essential amino acids( tyrosine, glutamine, asparagine, and proline) in the serum of patients in the NMOSD group were lower than those in the MOGAD group, and the differences were statistically significant( t/Z=2.184, 2.463, 2.805, 2.351; all P < 0.05). The levels of serum tryptophan, valine, and tyrosine in patients with NMOSD were statistically negatively correlated with EDSS scores( r=-0.318, -0.248, -0.242; all P< 0.05). Conclusions Lower overall amino acid levels are one of the key reasons why patients with NMOSD experience more severe neurological disability than those with MOGAD. Paying attention to the metabolic and nutritional status of patients with NMOSD and supplementing them with essential amino acids play a positive role in improving their condition and halting disease progression.
