    摘要:

    目的 探究伴自杀未遂青少年抑郁症患者静息态功能磁共振成像(rs-fMRI)的低频振荡 振幅(ALFF)和比率低频振荡振幅(fALFF)的变化及其意义。方法 本研究为横断面研究。选取苏州 市广济医院2022 年10 月—2023 年7 月住院的青少年抑郁症患者65 例为研究对象,并根据其近1 年内 有无自杀未遂史,将其分为伴自杀未遂青少年抑郁症组(SD 组,n=32)与不伴自杀未遂青少年抑郁症组 (ND 组,n=33),并同期通过社区招募31 名健康个体作为健康对照组(HC 组)。所有受试者均进行rs-fMRI 扫描,并基于影像数据计算出ALFF和fALFF值,抑郁严重程度采用患者健康问卷(PHQ-9)进行评估, 自杀风险通过简明儿童少年国际神经精神访谈(儿童版)(MINI-Kid)中的自杀评估版块进行评定,采用 方差分析比较3 组间的差异,并进行事后检验;在此基础上,采用Spearman 相关分析进一步分析差异脑 区的ALFF、fALFF 值与PHQ-9、MINI-Kid 自杀版块得分的相关性。结果 与HC 组比较,SD 组右侧额 上回ALFF值及左侧额上回、双侧额中回、左侧缘上回fALFF值均升高,而左侧中央前回的fALFF值降 低,差异有统计学意义(均体素水平P< 0.001,团块水平P< 0.05,GRF校正);ND 组左侧角回ALFF值及 右侧额中回、左侧缘上回、左内侧、旁扣带脑回fALFF值升高,但双侧枕上回fALFF值降低(均体素水平 P < 0.001,团块水平P< 0.05,GRF校正)。与ND组比较,SD组左侧枕上回的fALFF值升高,左侧岛叶 和右侧中央后回的fALFF值降低(均体素水平P< 0.001,团块水平P< 0.05,GRF校正)。相关性分析结 果显示,SD 组左侧缘上回fALFF 值与PHQ-9 评分呈负相关(r=-0.469,P=0.007)。结论 伴自杀未遂的 青少年抑郁症患者多个脑区活动异常,且左侧缘上回fALFF值与青少年抑郁症的严重程度呈负相关。

    Abstract:

    Objective To investigate the changes in amplitude of low-frequency fluctuations( ALFF) and fractional amplitude of low-frequency fluctuations( fALFF) in resting-state functional magnetic resonance imaging( rs-fMRI) among depressive disorder adolescents with attempted suicide, and to explore their clinical significance. Methods This study was a cross-sectional study. A total of 65 depressive disorder adolescents admitted to Suzhou Guangji Hospital from October 2022 to July 2023 were selected as research subjects. According to whether there was a history of attempted suicide within the past year, patients were divided into an adolescent depressive disorder group with attempted suicide( SD group,n=32) and an adolescent depressive disorder group without attempted suicide( ND group,n=33). At the same time, 31 healthy individuals were recruited from the community as a healthy control group( HC group). All participants underwent rs-fMRI, and ALFF and fALFF values were calculated based on the imaging data. The severity of depressive disorder was assessed using the Patient Health Questionnaire-9( PHQ-9), and suicide risk was evaluated using the suicide subscale of the Mini-International Neuropsychiatric Interview for Children and Adolescents( MINIKid). Analysis of variance was used to compare differences among the three groups and to conduct post-hoc tests. On this basis, Spearman correlation was used to analyze the correlation between ALFF and fALFF values in different brain regions and PHQ-9 scores and MINI-Kid suicide subscale scores. Results Compared with the HC group, patients in the SD group exhibited statistically significant increases in ALFF values in the right superior frontal gyrus and in fALFF values in the left superior frontal gyrus, bilateral middle frontal gyrus, and left supramarginal gyrus, while fALFF values in the left precentral gyrus were statistically reduced( all voxellevel P< 0.001, cluster-level P< 0.05, GRF-corrected). In the ND group, there were statistically significant increases in ALFF values in the left angular gyrus and in fALFF values in the right middle frontal gyrus, left supramarginal gyrus, left medial gyrus, and parahippocampal gyrus, as well as a statistically significant decrease in fALFF values in the bilateral supratemporal gyrus( all voxel-level P<0.001, cluster-level P<0.05, GRF-corrected). Compared with the ND group, the patients in the SD group showed elevated fALFF values in the left supratemporal gyrus and reduced fALFF values in the left insula and right postcentral gyrus, with all differences being statistically significant( all voxel-level P< 0.001, cluster-level P< 0.05, GRF-corrected). Correlation analysis revealed that the fALFF values in the left supramarginal gyrus of patients in the SD group were negatively correlated with the PHQ-9 scores, and this difference was statistically significant( r=-0.469, P=0.007). Conclusions Depressive disorder adolescents with attempted suicide exhibit abnormal brain activity in multiple regions, and the fALFF values in the left superior marginal gyrus are negatively correlated with the severity of adolescent depressive disorder.
