    目的 了解公众对精神分裂症态度的现状并分析其相关影响因素。方法 本研究为横断 面调查,于2024 年7—9 月采用方便抽样法,选取北京市、天津市、河北省无精神障碍诊断史的712 名健 康受试者为研究对象。基于“生物-心理-社会”理论,将精神分裂症重新命名为药物相关的精神疾病(命 名1)、焦虑相关的精神疾病(命名2)、创伤相关的精神疾病(命名3)、应激敏感相关的精神疾病(命名4)。采 用一般资料调查表收集患者的一般资料、采用精神分裂症态度问卷评估健康受试者对精神分裂症的态 度。采用多重线性回归分析一般资料、命名方式等对疾病态度的影响。结果 本研究共发放问卷712份, 获得有效问卷680 份,问卷有效回收率为95.6%。不同年龄、认识精神分裂症患者情况和医学生年级的 受试者,在原命名组精神分裂症态度问卷的总分比较,差异均有统计学意义(Z=-2.22、-4.33、-2.52,均 P< 0.05);原命名组分别与命名2 组、命名3 组受试者精神分裂症态度问卷总分比较,差异均有统计学 意义(Z=-6.62、-8.22,均P< 0.05);原命名组分别与命名1、2、3、4 组受试者精神分裂症态度问卷不同条 目评分比较,条目8 评分的组间比较均无统计学意义(均P > 0.05),命名1 组条目1、命名3 组条目4、命 名4 组条目3 评分组间比较也均无统计学意义(均P> 0.05),其余条目评分组间比较均有统计学意义(均 P< 0.05);将受试者按不同人群类型分组后,3 组受试者精神分裂症态度问卷的总分(H=36.57)和条目 1~8 评分(H=107.94、49.40、89.53、76.01、44.35、12.83、38.59、62.71),组间比较差异均有统计学意义(均 P< 0.05);多重线性回归分析结果显示,医学生、精神卫生专业人员、应急敏感相关精神疾病、药物相关 精神疾病、创伤相关精神疾病是精神分裂症态度问卷总分的预测变量(β=1.58、2.07、0.50、0.12、0.16,均 P<0.05)。结论 公众对精神分裂症的态度不受性别与受教育程度影响;年龄、以“生物-心理-社会” 为基础的命名方式、不同职业和教育背景、是否接触精神分裂症患者均可不同程度影响疾病态度。

    Abstract:

    Objective To explore the current status of public attitudes towards schizophrenia and analyze its influencing factors. Methods This study was a cross-sectional survey. From July to September 2024, convenience sampling was used to select 712 healthy participants without a diagnosis of mental disorders in Beijing, Tianjin, and Hebei as research subjects. Based on the "bio-psycho-social" theory, schizophrenia was renamed as the drug-relate psychosis( name 1), anxiety-related psychosis( name 2), trauma-related psychosis( name 3), and stress sensitivity-related psychosis( name 4). The General Information Questionnaire was used to collect participants' general information. The Schizophrenia Attitudes Questionnaire was used to assess healthy participants' attitudes toward schizophrenia. Multiple linear regression was used to analyze the influence of general characteristics and naming conventions on attitudes toward the disease. Results A total of 712 questionnaires were distributed, and 680 valid questionnaires were obtained, resulting in a valid response rate of 95.6%. There were statistically significant differences in the total scores on the Schizophrenia Attitudes Questionnaire of the original name group among participants of different ages, levels of familiarity with schizophrenia patients, and grades of medical students( Z=-2.22,-4.33,-2.52; all P<0.05). There were statistically significant differences in the total scores of the Schizophrenia Attitudes Questionnaire between the original name group and the name 2 group and the name 3 group( Z=-6.62, -8.22; P<0.05). In the comparison of scores for different items of the Schizophrenia Attitudes Questionnaire among participants in the original name group and name groups 1, 2, 3, and 4, there were no statistically significant differences in the scores for item 8 between the groups( all P>0.05). Similarly, there were no statistically significant differences in the scores for item 1 in the name 1 group, item 4 in the name 3 group, and item 3 in the name 4 group between the groups( all P> 0.05). However, there were statistically significant differences in the scores for the remaining items between the groups( all P<0.05). After grouping the participants according to different population types, the inter-group comparisons of the total scores( H=36.57) and item scores 1-8( H=107.94,49.40,89.53,76.01,44.35,12.83, 38.59, 62.71) of the Schizophrenia Attitudes Questionnaire among the three groups were statistically significant (all P< 0.05). Multiple linear regression analysis revealed that medical students, mental health professionals, emergency-sensitive related mental disorders, drug-related mental disorders, and trauma-related mental disorders were predictors of the total score on the Schizophrenia Attitudes Questionnaire, with statistically significant differences( β=1.58, 2.07, 0.50, 0.12, 0.16; all P<0.05). Conclusions Public attitudes toward schizophrenia are not influenced by gender or educational attainment. Age, naming conventions based on "biopsycho- social", different occupations and educational backgrounds, and whether or not one has contact with schizophrenia patients can all influence attitudes toward the illness to varying degrees.
