    摘要:

    目前抑郁症的临床治疗仍存在诸多困境:常规药物治疗普遍有效率不足,而电休克疗法 易给患者带来认知方面的不良反应。这一现状促使以精准性、安全性、个体化为核心特征的无创神经 调控技术(NIBS)得到快速发展。磁共振成像(MRI)可同步完成脑区功能解析与解剖结构定位,这一关 键技术为引导精准靶向干预提供了可靠的工具。依托MRI 引导,重复经颅磁刺激(rTMS)、经颅聚焦超声 刺激(tFUS)、经颅时域干涉电刺激(tTIS) 3 类干预技术,正促使抑郁症治疗模式摆脱传统经验式定位,逐 步向个体化精准靶向调控转变。现全面梳理了上述MRI引导的调控技术,分析其在抑郁症治疗领域的 研究现状与最新进展,同时指出该领域现阶段存在的关键挑战和难题。未来,闭环调控系统的研发落地、 多模态数据的深度融合搭配人工智能辅助决策、标准化数据处理流程搭建及多中心临床试验验证的推 进,都会成为该领域后续发展的主要方向。伴随相关技术的不断迭代升级,结合多模态影像与人工智 能制定的个性化治疗方案,将有希望革新现有的抑郁症治疗模式,助力精神医学领域从经验化诊疗向 精准化诊疗的转型。

    Abstract:

    The clinical treatment of depressive disorder has long faced core challenges, such as limited drug efficacy and cognitive side effects associated with electroconvulsive therapy. This situation has driven the rapid development of non-invasive brain stimulation techniques( NIBS) techniques characterized by precision, safety, and individualization. Magnetic resonance imaging( MRI), as a key technological carrier that bridges the brain function mapping and anatomical localization, provides a reliable technical pathway for guiding precisetargeted interventions. Among these, the three representative technologies of repetitive transcranial magnetic stimulation( rTMS), transcranial focused ultrasound( tFUS), and transcranial temporal interference stimulation (tTIS) are driving the transformation of depressive disorder treatment from conventional experiential localization to individualized and precise-targeted neuromodulation paradigm. This paper systematically reviews the current state and progress of MRI-guided modulation techniques for the treatment of depressive disorder, while also highlighting key challenges in this field. Looking ahead, the development of closed-loop control systems, deep integration of multimodal data with artificial intelligence-assisted decision-making, and the advancement of standardized processes and multicenter validation will become the core directions for development in this field. As technology continues to evolve, personalized modulation strategies based on multimodal imaging and artificial intelligence are expected to revolutionize the treatment paradigm for depressive disorder, ultimately propelling psychiatry from empirical medicine toward precision medicine.
