    目的 分析梦魇障碍的青少年发生非自杀性自伤(NSSI)行为的情况和风险因素。 方法 本研究为横断面研究。选取2022 年5 月1—30 日山东省莒县的5 所高中和5 所初中的在校学生 进行问卷调查,以问卷星的形式发放问卷,发放7 734 份问卷,回收有效问卷7 002 份,有效回收率为 90.54%。筛选符合入组条件的青少年共6 700 例,根据梦魇障碍指数问卷中文版(NDI-CV)评分,第1 项 回答为0,为无梦魇障碍组(n=3 674),第1 项回答不是0,且第2~4 项回答均为0 或1,为存在阈下或部分 梦魇障碍症状组(n=2 812),第1 项回答不是0,第2~4 项的回答≥ 2,为可能梦魇障碍组(n=214),分别比 较不同梦魇障碍状况的青少年的一般资料、青少年非自杀性自伤行为量表(ANSAQ)评分,并进行事后 两两比较。记录可能梦魇障碍青少年NSSI行为的发生情况,分为有NSSI 组(n=125)和无NSSI 组(n=89), 采用NDI-CV、失眠严重程度指数量表(ISI)、青少年日间嗜睡量表(CADSS)、患者健康问卷(PHQ-4)、自我 厌恶量表(SHS)、童年不良经历问卷修订版(ACEQ-R)记录青少年的睡眠问题、情绪状况、自我厌恶和童 年不良经历等得分。采用多因素Logistic 回归分析青少年NSSI 的影响因素,构建风险预测模型,绘制受 试者工作特征(ROC)曲线,验证模型的预测效能。结果 无梦魇障碍组ANSAQ 总分为0(0,1.00)分,阈 下或部分梦魇障碍症状组ANSAQ 总分为0(0,1.00)分,可能梦魇障碍组ANSAQ 总分为6.50(0,17.00)分, 3 组比较差异有统计学意义(H=566.380,P< 0.001),事后两两比较差异均有统计学意义(均P < 0.001)。 单因素分析结果显示,有NSSI 组和无NSSI 组青少年的ISI、PAQ-4、SHS、ACEQ-R 得分比较,差异有统计 学意义(Z=-2.174、-6.046、-12.671、-6.312;均P< 0.05)。多因素Logistic 回归分析显示,焦虑抑郁情绪 (OR=1.170, 95%CI:1.011~1.354,P=0.035)、自我厌恶(OR=1.084,95%CI:1.047~1.123,P < 0.001)、童 年不良经历(OR=1.232, 95%CI:1.053~1.441, P=0.009)均为梦魇障碍的青少年发生NSSI 行为的独立危 险因素。根据多因素Logistic 回归分析结果构建梦魇障碍的青少年发生NSSI行为的风险预测模型,结 果显示,风险预测模型的曲线下面积为0.838(95%CI:0.783~0.894,P< 0.05),敏感度为72.0%,特异度 为85.4%,预测效能较好。结论 梦魇障碍青少年NSSI行为的发生率较高,且受到多因素的影响,焦虑 抑郁情绪、自我厌恶、童年不良经历均为危险因素,根据危险因素构建的预测模型对NSSI 行为的发生具 有较好的预测价值。

    Abstract:

    Objective To analyze the situation and risk factors of non-suicidal self-injury( NSSI) behavior in adolescents with nightmare disorder. Methods This study was a cross-sectional study. A questionnaire survey was conducted among students enrolled in five high schools and five middle schools in Ju County, Shandong Province, from May 1 to 30, 2022. The questionnaires were distributed via the Wenjuanxing platform. A total of 7 734 questionnaires were distributed, and 7 002 valid questionnaires were recovered, with an effective recovery rate of 90.54%. A total of 6 700 cases that met the inclusion criteria were selected. Based on scores of the Chinese version of the Nightmare Disorder Index( NDI-CV), participants whose answer to Item 1 was "0" were classified as the non-nightmare disorder group( n=3 674); those whose answer to Item 1 was not "0" but answers to Items 2-4 were "0" or "1" were classified as the subthreshold or partial nightmare disorder symptom group( n=2 812); those whose answer to Item 1 was not "0" but whose answers to Items 2-4 were "2" or higher were classified as the possible nightmare disorder group( n=214). The general information and Adolescent Non-suicidal Self-injury Assessment Questionnaire( ANSAQ) scores of adolescents with different nightmare disorders were compared respectively, and post-hoc pairwise comparisons were conducted. The incidence of NSSI behavior in adolescents with possible nightmare disorder was recorded and divided into the NSSI group( n=125) and the non-NSSI group( n=89). The NDI-CV, Insomnia Severity Index( ISI), Chinese Adolescent Daytime Sleepiness Scale( CADSS), Patient Health Questionnaire-4 items( PHQ-4), Self- Hate Scale( SHS), and Adverse Childhood Experiences Questionnaire-Revised( ACEQ-R) were used to record adolescents' scores on sleep problems, emotional status, self-hatred, and adverse childhood experiences. Multivariate Logistic regression was used to analyze the influencing factors of NSSI in adolescents, and the risk prediction model was constructed. The receiver operating characteristic( ROC) curve was drawn to validate the predictive performance of the model. Results The ANSAQ total scores for the non-nightmare disorder group, the subthreshold or partial nightmare disorder symptoms group, and the possible nightmare disorder group were 0(0,1.00), 0(0,1.00), and 6.50(0,17.00), respectively, there were statistically significant differences in ANSAQ scores among adolescents in different nightmare disorder groups( H=566.380,P < 0.001), and all post-hoc pairwise comparisons were statistically significant( all P< 0.001). One-way analysis revealed statistically significant differences between the NSSI group and the non-NSSI group in terms of scores for ISI, PHQ-4, SHS,ACEQ-R (Z=-2.174,-6.046,-12.671,-6.312; all P < 0.05). Multivariate Logistic regression analysis showed that anxiety and depression[ OR=1.170, 95%CI( 1.011,1.354),P=0.035], selfhatred [ OR=1.084, 95%CI( 1.047, 1.123),P< 0.001], adverse childhood experiences[ OR=1.232, 95%CI (1.053,1.441),P=0.009] were all independent risk factors for NSSI behavior among adolescents with nightmare disorder, and these differences were statistically significant. A risk prediction model for NSSI behavior among adolescents with nightmare disorder was constructed according to multivariate Logistic regression analysis. ROC curve analysis showed that the area under the curve for the risk prediction model was 0.838[ 95%CI( 0.783, 0.894), P < 0.05], with the sensitivity of 72.0% and the specificity of 85.4%, indicating good predictive performance. Conclusions The incidence of NSSI behavior in adolescents with nightmare disorder is high, and is affected by multiple factors. Anxiety and depression, self-hatred, and adverse childhood experiences were risk factors. The prediction model based on risk factors has good predictive value for the occurrence of NSSI behavior.
